How do I submit a Referral for one of my Patients?

Submit a Guest Referral

SC Note: Guest Referrals cannot be edited and will not appear in the referring practice's portal account since they do not require logging in. These are not the preferred method of referring cases to our hospitals but are an option to replace "paper" referrals. 

1. Click on the Patient Referrals Tab
2. Click Refer Patient
3. Complete the Referral Fields
4. Attach any additional Records or Images
5. Click Submit

Log in to the Portal using your practice's login credentials

Referrals

Once you have logged into the portal, you will be able to see records for any mutual patients. You will see any Referrals previously submitted and you can edit them at any time and changes or additions resubmitted.

Submit a Referral

1. Click on the Patient Referrals tab
2. Click on the Refer Patient Tab
3. Complete the fields within the Online Referral Form
4. Attach any/all records, diagnostics, images, etc...
5. Click Submit  

Edit a Referral

1. Click on the Patient Referrals Tab
2. Use the Search Window to enter the patient or referral information
3. Select the Referral you wish to edit
4. Make your changes
5. Click Submit
